Registration for the conference is $235.

The fee includes:

• Continental Breakfasts (Fri. & Sat.)
• Lunches (Fri. & Sat.)
• Dinner (Fri. only)

Due to venue space restrictions, participation is limited to 225 people. Registration is on a first-come-first-served basis.

Registration Payment*

You can pay for your registration in one of two ways:

1. By check. Please make the check (in the amount of $235) payable to LOEX Clearinghouse. Mail the payment to: LOEX Clearinghouse, Halle Library, Eastern Michigan University, Ypsilanti, MI 48197-2207.

2. By credit card (Visa and Mastercard only). We require the following information: authorization for the amount ($235), the card type (Visa or MC), the card number, the expiration date, and the name on the card. You can phone this information to 1.734.487.0020, x2200 or fax it to 1.734.484.1151.

Once we receive your payment, we will e-mail you a receipt and note that you have paid on our list of registrants. Please allow us at least 3 working days to send you a receipt.

If we do not receive payment by 5:00 p.m. (ET) on March 31, we will have to give your space to someone on our waiting list.

Important Dates

• Conference Registration: February 13, 2004 (LOEX members)
                                        March 12, 2004 (Non-LOEX members if slots remain.)
• Registration Payment* due: March 31, 2004 by 5:00 p.m. ET
• Conference Dates: May 6 - 8, 2004

* LOEX can accept personal and institutional checks, and VISA or MasterCard as payment. We cannot accept purchase orders.
